Practice Plan: Supporting you with the CQC A guide to the application to be registered with the Care Quality Commission All providers of dental care will need to be registered with the Care Quality Commission (CQC) on 1st April 2011 in order to continue to practise legally. In order for this to be guaranteed, those dental practitioners who need to register will be asked to complete an application form and submit it during an allocated window that will end no later than 31st December 2010. Practice Plan has developed some guidance notes to assist you with the completion of the form, which has been reviewed by the CQC and can be downloaded from this page. In drafting this guidance we have consulted with and obtained guidance from the CQC. Feel free to print this guide for your own reference, but remember that the links suggested in the guide will be very easy to access from the electronic guide itself (one click and you are there!). Please remember that the specific cases cited in this guide are only examples of how to approach answering the questions in the registration form and should be seen as helpful guidance only.
